The material in Segment 8 focuses on the Staff and Office Files of Arnold Epstein, Charlotte Hayes, Paul Jamieson, Kelcey Kintner, Amanda Merryman, Meeghan Prunty, and Jason Solomon. The records include reports, drafts of speeches, talking points, press releases, memoranda, newspaper articles, handwritten notes, statistical charts, publications, and correspondence. The documents highlight the history of health care, the “Health Care Express Tour,” and other proposed health care legislation. Scope and Content Note The materials in FOIA 2006-0885-F (segment 8) are a selective body of documents responsive to the topic of the FOIA. Researchers should consult the archivist about related material. Additional materials for this FOIA case have been processed in segments 1-7. FOIA 2006-0885-F (segment 8) contains records of the Clinton Administration’s efforts to complete a major transformation of the health care system in the United States. This segment is composed of the records of Arnold Epstein, Charlotte Hayes, Paul Jamieson, Kelcey Kintner, Amanda Merryman, Meeghan Prunty, and Jason Solomon. Included in the material are reports, briefing books and documents, statistics, numerous newspaper articles and papers from scholarly journals, correspondence, preliminary assessments of major health care issues, evaluations of health reform bills put forward by both Democrats and Republicans, and drafts of portions of bills associated with overhauling the entire process of health care. The two major administration figures tasked with addressing the problem of health care reform were First Lady Hillary Rodham Clinton and Ira Magaziner. Bill Clinton held a press conference in late January of 1993 at which he announced the formation of the President’s Task Force on National Health Care Reform, more commonly known as the Health Care Task Force or HCTF. Hillary Clinton chaired the HCTF, while Magaziner, acted as chief policy advisor. Other administration officials serving on the HCTF included Secretaries of the Treasury, Health and Human Services, Commerce, Labor, Defense, and Veterans Affairs; also part of the group were the Director of the Office of Management and Budget, the Assistants to the President for Domestic Policy and Economic Policy, and the Chair of the Council of Economic Advisors. The end product of the HCTF took the form of the Health Security Act of 1993. Debated in Congress for the better part of a year, this piece of legislation was never put to a vote. Key components of the proposed law went as follows: managed competition, competition between insurers, government regulation to keep health care costs affordable, universal coverage, and employer mandates. Congress took up other assorted bills for consideration (the Senate Finance Committee bill, the Mitchell bill, the Chafee bill, the Cooper-Breaux bill, and Senator Dole’s bill), but passed nothing in the way of meaningful reform. All of the responsive records for segment 8 of 2006-0885-F were found in the Staff and Office files of staffers involved in the issue of health care reform. This segment encompasses a wide array of materials ranging from deliberative and substantial memoranda to publications. The records of Arnold Epstein, a medical doctor, consist principally of articles, reports, papers, publications, presentations, statistics, talking points, and drafts of bills. Within these documents topics (that would appear throughout the collection) include the role of academic health centers in health care reform, public and workforce initiatives in the Health Security Act of 1993, insuring quality of care in the new system, and graduate education for nurses and other professionals. On many occasions material would take the form of presentations, or perhaps more accurately talking points, highlighting key features in the move for drastic renovation of health care as it existed in the United States. The records of Charlotte Hayes contains material largely related to policy options for the health care of minorities (African Americans, Hispanics, Native Americans). For instance, the folders on African Americans have handwritten notes (apparently by Ira Magaziner) concerning possible problems with Medicaid in that particular community. A paper entitled “HIV/AIDS and Hispanics of the Southwest: Recommendations for Change” can be readily located, while “The Clinton Health Care Plan and Indian Health” ,a 10 page report, contains analysis and attention to detail. The records of Paul Jamieson, on loan from the Department of Education to the HCTF, are largely composed of newspaper articles, correspondence, memoranda, papers, reports, charts, testimony, lists, and press releases. Several topics receive a considerable amount of coverage, perhaps none more than the history of health care reform in the United States. A twentieth century phenomenon beginning with FDR, transforming the delivery of health care has always been a divisive issue among Americans. Numerous drafts of reports, photocopies of newspaper articles from the 1960s, and excerpts from The Public Papers of the Presidents of the United States can be found here and help put everything about the matter in historical context. Other subjects include international comparisons of health care systems, the scoring of Clinton’s Health Security Act of 1993 by the CBO (Congressional Budget Office), and whether or not price controls on health insurance and drugs were an explicit part of the Clinton Administration’s plan to bring change to how Americans consulted their doctors. In Kelcey Kintner’s materials, schedules, faxes, drafts, press releases, reports, and newspaper articles detail health care reform issues. Members of Clinton’s Cabinet took an active role in educating the American public regarding the major changes in health care delivery. Secretary of the Treasury Lloyd Bentsen, Secretary of the Interior Bruce Babbitt, Deputy Secretary of the Treasury Roger Altman, Secretary of Veterans Affairs Jesse Brown, Secretary of Health and Human Services Donna Shalala, and Secretary of Housing and Urban Development Henry Cisneros all made appearances and gave speeches in support of legislation sponsored by the President and First Lady then being debated in Congress. In addition, Kintner’s records document what took place during the “Health Care Express Tour,” a bus caravan that traversed the United States and engaged in an assortment of activities dedicated to highlighting the health care plan. The files of Amanda Merryman contain drafts of portions of the Health Security Act of 1993. Often times these records show handwritten revisions, suggestions, and objections scrawled on the drafts. Each folder comes with a table of contents for that particular issue (such as coverage and responsibilities, long-term care: home and community-based services, etc.). More than half the collection can be found in the records of Meeghan Prunty, who played a major role in formulating policy for health care reform while occupying the position of Associate Director of Research and Strategic Planning from 1993 to 1995. These files contain correspondence, handwritten notes, publications, testimony, newspaper articles, fact sheets, memoranda, briefing papers, drafts, statements, reports, papers, speeches, charts, press releases, talking points, articles from scholarly journals, agendas, faxes, and schedules. Organizations highlighted (describing their position on Clinton health care reform) include the AMA (American Medical Association), NFIB (National Federation of Independent Business), AHA (American Hospital Association), Godfather’s Pizza, and Starbucks Coffee Co. Important topics covered include academic health centers and their role in changing the health care delivery system, proposed legislation (besides the Health Security Act)—Gephardt bill, Mitchell bill, Dole bill, Michel bill, and Rowland-Bilirakis bill, how the Congressional Budget Office scored each act, and events connected with promoting various aspects (cost containment, financing, Medicare, job creation, etc.) of the plan advanced by the Clinton Administration. In addition, there are papers dealing with the stance of key individuals (Alain Enthoven and Victor Fuchs, for example) on Clinton’s health care reform proposal and analysis of proposed savings for every state. The files of Jason Solomon are comprised of memoranda, newspaper articles, charts, papers, reports, graphs, press releases, drafts, correspondence, outlines, bulletins, and handwritten notes. Much attention is given to alternative plans. A paper entitled “The Dole Plan: No Alternative for Middle Class Families” and a report prepared for SEIU (Service Employees International Union) by Lewin-VHI, Inc. with the heading of “The Impact of Senator Dole’s Health Reform Plan on States” highlight perceived weaknesses and shortcomings of these alternative plans. Mitchell’s bill receives praise in the records of Solomon. A one page paper stresses the advantages of “The Mitchell Approach”—“Guarantees Health Coverage You Can Never Lose,” “Protects Families from the Insurance Companies,” and “Helps Older Americans.” Record Type Textual System of Arrangement Records that were responsive to this FOIA request were found in one collection area—Clinton Presidential Records: White House Staff and Office Files. Staff and Office Files were maintained at the folder level by staff members within their individual offices and document all levels of administration activity. Staff and Office Files are processed at the folder level, that is, individual documents are not selected and removed from a folder for processing. While this method maintains folder integrity, it frequently results in the incidental processing of documents that are not wholly responsive to the subject area.
